    The San Francisco coach was involved in a head-on collision with another driver on July 14 in Palo Alto, CA and, as NBC Sports Bay Area’s Matt Maiocco reported Saturday, suffered numerous injuries, including a concussion, broken ribs, a broken nose and a broken hand.

    Public records from the California Crash Reporting System, a statewide database of collisions, show the accident occurred at 6:12 p.m. PT at Alma Street and Hamilton Avenue after a 21-year-old woman made an unsafe turn while driving a Mercedes SUV in the opposite direction on a four-lane, 25-mph street.     Shanahan was driving south on Alma Street, while the other driver was headed north.

    The records also show that Shanahan was holding and using a cellphone at the time, which authorities listed as a factor in the accident, but stated that he was not at fault for the wreck.

    According to the database, neither Shanahan nor the woman were found to have been drinking, and both parties were wearing seat belts.

    Palo Alto police Lt. Nicholas Martinez told the San Francisco Chronicle on Saturday that the officers who responded to the accident did not issue any citations or suspect either driver of being impaired and that both parties remained at the scene and cooperated with police.
